Join us on Microsoft Teams to hear about the development and impact of the NHS West Yorkshire Integrated Care Board (ICB)’s Unpaid Carers Hospital Discharge Toolkit.

This event is for anyone who has an interest in projects to improve the experience of unpaid carers, those with an interest in improving the experience of patients / unpaid carers at the point of discharge from health settings and those with an interest in projects that promote integrated working.  The event will provide an opportunity to hear about the development of the toolkit and its impact.

The toolkit is intended for all areas of discharge including acute hospitals, mental health hospitals, and virtual wards.  

It has been coproduced with NHS colleagues, voluntary sector carer organisations and people with lived experience, including those from ethnically diverse backgrounds and young carers.
