NEY: UEC Hospital Discharge Improvement Initiative Discharge: Regional wide event

In advance of Winter the North East and Yorkshire UEC team is running a series of clinical improvement programmes.  The Hospital Discharge Improvement Initiative will focus on the practical steps that can be taken to reduce in hospital delays to discharge

The programme is aimed colleagues working in providers and systems on Acute Hospital Discharge and is designed to support you deliver on the improvement plans that have already been agreed within your local systems.
