Improving hospital discharge for people with dementia

Evidence indicates that people with dementia:

• Occupy 25% of acute hospital beds
• Stay in hospital twice as long as others over age 65
• Are often admitted due to urinary tract and chest infections treatable in the community (43%)

Join the NHS England Dementia Policy team for a webinar about improving the hospital discharge process for people with dementia. Transitioning from hospital to home or long-term care can be a challenging journey for people living with dementia, their families, and healthcare providers. This session will explore practical strategies to streamline the discharge process, reduce length of stay, and improve patient outcomes.
