This rapid improvement event will explore:
children & young people autism/neuro-disability pathways
pre/post diagnosis support
Aims of the event:
This event will explore the distinct care pathways for children and young people with Autism and more widely neuro-disabilities.
With an aim to better understand the role of education, health and social care.
You will be asked to;
- Capture information regarding the commissioning and delivery of effective, integrated care pathways
- Identify and share good practice, shared risk and gaps in the provision of effective, responsive care pathways
- Identify improvements in joint working throughout a child or young person’s journey in their care pathway
- Agree a way forward to ensure we capitalise on the above outcomes.
Who should attend?
This event is aimed at people with lived experience, carers and professionals. Along with people who have a role in commissioning and providing education, health and social care support for children and young people with Autism and Neuro Disabilities.
This may include:
- SEND designated Medical and Clinical Officers
- SEND South West Network Forum
- Commissioners and leaders from children’s health, education and social care
- Parent Carer Forums
- South West Clinical Network
