Improving hospital discharge for people with dementia
Evidence indicates that people with dementia:
- Occupy 25% of acute hospital beds
- Stay in hospital twice as long as others over age 65
- Are often admitted due to urinary tract and chest infections treatable in the community (43%)

Transitioning from hospital to home or long-term care can be a challenging journey for people living with dementia, their families, and healthcare providers.
